When you are using this form to request a reset of your license ID you agree with the following:
1. You acknowledge that the above license must be reset in connection with a change of hardware (components) or OS and you declare that the above license will be used for the same customer and location.
2. You agree to the costs of this license reset. (if applicable, you will be notified first).
3. You agree to immediately pay the cost for a similar license ID if Griffid registers that the license ID mentioned above is used for purposes other than the above destination (change of hardware).
